Phase 39: Provider Adapter Refactor

- ProviderAdapter trait with chat(), embed(), unload(), health()
- OllamaAdapter wrapping existing AiClient
- OpenRouterAdapter for openrouter.ai API integration
- provider_key() routing by model prefix (openrouter/*, etc)
